What happens if a purchase that I made is not properly tracked by the Fluz app, and I do not get credited with my cash back reward?

In the rare case that the app failed to distribute cash back earnings, users will be able to simply fill out a short form right inside the app. This “lost and found” form will ask a few simple questions such as the merchant where the purchase was made, total purchase amount, designated cash back percentage as displayed in the app, the method of payment that was used to make the purchase, and the date.
Fluz will automatically pay users the omitted cash back reward based on the information in the form “no questions asked,” however, if a user is later found to have falsified the transaction in order to game the system, they will have their account permanently locked and any cash back rewards held within the account will be forfeited.
